The weekly NBA rankings are determined by opportunity and performance. For this week---ATL/DEN/DET/HOU/MIA/MEM/OKC/POR/TOR/UT all have just 2 games on the slate. The rest of the teams have 3 games for the week and that changes things a lot. Someone as dominant as James Harden is moved down 10 spots because of the lack of third game (and other drama surrounding him being in Houston). 

A point system is used that weighs specialties like steals, blocks and three-point shooting a bit heavier. Percentages are not looked at as strongly, but are still taken into consideration.
